More accurate nets, fees, and refunds

We improved cart financial accuracy by calculating card fees and net paid amounts more reliably, even when payments included multiple charges or adjustments. Refund totals also pulled from explicit refund entries to prevent duplicates, so your reports and payout reconciliation matched what actually happened.
